Individual (Adolescent or Adult ) Empowerment Plan

3 month REGULAR support program:

  • 1 day “Family/Individual Empowerment Day”
  • 5 calls per month @ 45 minutes each
    • 4 calls with adolescent or adult
    • 1 call with family
  • 3 months ongoing structure and support
  • Re-evaluation upon program completion
  • This package includes 2 emergency calls.

6 month EXTENDED support program:

  • 1 day “ Family/Individual Empowerment Day”
  • 5 calls per month @ 45 minutes each
    • 4 calls with adolescent or adult
    • 1 call with family
  • 6 months ongoing structure and support
  • Re-evaluation meetings during month 3 and upon program completion
  • This package includes 3 emergency calls.

12 month INTENSIVE support program:

  • 1 day “ Family/Individual Empowerment Day”
  • 5 calls per month @ 45 minutes each
    • 4 calls with adolescent or adult
    • 1 call with family
  • 12 months ongoing structure and support
  • Re-evaluation meetings every 12 weeks and upon program completion
  • This package includes 4 emergency calls.

Family Empowerment Plan

Family empowerment day was amazing. It brought out all the issues that seemed to be roadblocks to moving past the old cycles of anger and hurt.
